Why is my car slow to start in the winter?

Why does my 2018 BMW 120D Sport (with 21,000 miles) seem hesitant to start on icy cold mornings?

It'll almost certainly be down to the car's battery losing charge. Batteries often lose charge when it's very cold and an older battery may not hold sufficient charge to get the car running. We would give the car a good drive of 20 minutes or more to get the battery charged up, but if the issue still occurs then you'll likely need a new battery.
